Cagrilintide

Cagrilintide is a long-acting synthetic peptide that mimics amylin, a hormone co-secreted with insulin by pancreatic beta cells in response to food intake

It acts as a dual agonist for amylin and calcitonin receptors, primarily being investigated for its potential in treating obesity and type 2 diabetes

Amylin Receptor Activation: Cagrilintide binds to amylin receptors in the brain (e.g., hypothalamus, area postrema), promoting satiety, reducing appetite, and slowing gastric emptying, which prolongs feelings of fullness and reduces caloric intake

Calcitonin Receptor Engagement: It also activates calcitonin receptors, contributing to energy balance and potentially enhancing weight loss effects

Glucose Regulation: By suppressing glucagon secretion and slowing gastric emptying, cagrilintide helps stabilize postprandial blood glucose levels, benefiting those with type 2 diabetes

Long-Acting: Unlike natural amylin, which degrades quickly, cagrilintide is modified (e.g., via fatty acid acylation) to resist enzymatic breakdown, allowing once-weekly subcutaneous injections. This improves patient adherence compared to daily therapies like pramlintide

Combination Therapy: Often studied with Retatrutide or Semaglutide (GLP-1 receptor agonists), marketed as CagriReta or CagriSema, to enhance weight loss and glycemic control through complementary mechanisms

Research application

Copied from the handbook as research notes. and not instructions.

  • 5mg vial of Cagrilintide
  • Reconstitution volume: 2 mL of BAC water

Cagrilintide is typically administered once weekly via subcutaneous injection, with doses escalated every 4 weeks to reduce side effects like nausea

  • Weeks 1-4: 0.25 mg (10 units)
  • Weeks 5-8: 0.5 mg (20 units)
  • Weeks 9-12: 1.0 mg (40 units)
  • Weeks 13-16: 1.7 mg (68 units)
  • 10mg vial of Cagrilintide
  • Reconstitution volume: 2 mL of BAC water
  • .25mg (5 units)
  • .5mg (10 units)
  • 1mg (20 units)

Unless other wise noted intermuscular (IM) peptides are commonly administered via subcutaneous (SQ) injection with 30-31 gauge syringes
